Health Action Planning Goes Live Today!

We now have everything you need on the website to get you started with your health action plan.

You will find a complete health record and health action plan that you can type onto and print off.

Please start using them and take along to your health appointments.

Please encourage your GP or other health professionals to use them and remind them when you go for your annual health check.

There is also lots of information here for you about keeping healthy.

Our next development will be the community learning disability team information.

Website Launch

Our Website launch takes place on Monday 29th of June 2015. This will tell lots of people including GP’s, practice nurses, carers, community nurses and people with learning disabilities all about our site.
