We would like to welcome Emma, Graeme and Margaret! They will be promoting good health for people with a learning disability in Sunderland.
For example they will be helping people to get a health action plan and will support some people to have an annual health check.

The acute liaison service leaflet has been updated and added to the website today. Please see how the service can help you.

We now have everything you need on the website to get you started with your health action plan.
You will find a complete health record and health action plan that you can type onto and print off.
Please start using them and take along to your health appointments.
Please encourage your GP or other health professionals to use them and remind them when you go for your annual health check.
There is also lots of information here for you about keeping healthy.
Our next development will be the community learning disability team information.
